A law signed on December 23, 2024 changed how employers provide IRS Form 1095-C. Employers no longer have to mail this form to every employee. Instead, we must post a notice letting you know you can request a copy if you want one. This is that notice.
What is Form 1095-C?
Form 1095-C shows information about the health coverage your employer offered you. You do not attach it to your tax return, but you should keep it with your tax records.
What we will do
We will continue to provide Form 1095-C electronically to all active employees, as usual. If you would like a paper copy, we will mail one to you within 30 calendar days of request.
How to request a paper copy

In your email, include:
- Your full name
- A phone number where we can reach you
- Your current mailing address (please confirm it)
- A simple statement: "Please mail me a copy of my Form 1095-C"
