REQUIRED HEALTH FORMS
Every graduate student is required to complete and upload the documents listed below via the student health portal. Log in to the portal using the first part of your Sewanee email only (ex: abchelp0), leaving off @sewanee.edu and enter your password. Then enter your Banner ID for verification.
Once you have uploaded all of your documents, please be patient while a UHS staff member reviews your information. If something is missing or incomplete, you will be notified via Sewanee email.
Deadline for summer school graduate students is May 15.
Deadline for School of Theology students is July 31.
Health Form Checklist:
1. Health Form
- Demographics and Insurance
- Medical History
- Immunization Form filled out by a healthcare provider (only required if NO documentation of immunizations)
2. Immunizations (Still required to manually enter even after uploaded documentation)
- Hep B
- Measles
- Mumps
- Pertussis (Tdap)
- Rubella
- TB Risk Screening
- If you answer "yes" to any questions, you will be required to have one of the following:
- Negative TB Skin Test
- Negative QuantiFERON Gold Test
- Negative Chest XRay
- If you answer "yes" to any questions, you will be required to have one of the following:
- Varicella
- Meningococcal (only required for under 22 years old)
3. Add Insurance Card
- No need to fill out the information boxes- simply upload a picture of the front and back of your insurance card.
HOW TO MAKE A UHS APPOINTMENT
Registered graduate students are eligible to be seen at University Health Services. Appointments are required. Please call (931) 598-1270 to schedule. You may also make an appointment online via your student portal. UHS will file insurance for your visit or can arrange a discounted self-pay charge to your student account. Students are responsible for notifying their insurance provider of any changes of address so that coverage is not interrupted.
Counseling and psychological services (CAPS)
Graduate students who are full-time and degree-seeking are eligible for CAPS services. CAPS appointments are free and confidential.
New Clients: New clients (or previous clients who wish to meet with a different therapist) can schedule a 30-minute triage counseling appointment online or by calling the front desk at (931) 598-1325. To schedule online, visit your patient portal. Click on the "Appointments" tab, then select "Schedule an Appointment."
Returning Clients: If you saw a CAPS therapist in previous years and wish to continue working with that same therapist, OR if you have already been seen this academic year, you do not need to schedule a triage appointment. Please call the front desk at (931) 598-1325 to schedule.
